Case Managers Make A Difference
New York-Presbyterian Hospital/Columbia University Medical Center offers substance use services for adults, children and families receiving primary care in our Ambulatory Care Network clinics.  Many patients have complex biopsychosocial needs and require support in navigating medical and social systems.   This position will enable the program to extend the coordination and support for patients and families in the highest need to reduce barriers to care and enhance integration. 

Psychiatric Case Manager - Integrated Mental Health Program - Columbia – Days

Our Integrated Program Bridge Clinic is seeking a Psychiatric Case Manager to join our multi-disciplinary team providing care coordination, targeted referrals and case management, and supportive engagement for patients and their families presenting with emergent substance use disorder, co-morbid psychiatric and medical needs. 

Duties and responsibilities are aimed at Bridging Care between Emergency Department/Inpatient Hospitalization and the community for continued service access and improved outcomes in SUD population, including but not limited to access to Medication for Addiction Treatment (MAT), mental health services, medical services and appropriate level of substance use disorder care. 

Seeking candidates with strong patient engagement skills, efficient organizational skills, and investment in teamwork.  Position is full time and bilingual Spanish speaking candidates are encouraged to apply.

Preferred Criteria

  • Some training in behavioral health/psychology
  • Fluency in both English and Spanish
  • Experience working with adults, children, geriatrics, and/or families
  • Experience working with community-based organizations 

Required Criteria

  • Bachelor of Arts or Sciences Degree in Social Work, Psychology or equivalent from a recognized college or university.

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.

Salary Range:

$58,500-$79,000/Annual
