Case Manager & Hospice RN Liaison - Case Management - Hudson Valley - Full Time Days

At NewYork Presbyterian Hudson Valley Hospital (located in Cortlandt Manor, Upper Westchester County, NY), talented RN Case Managers lead, learn, and educate in a department like no other. We partner across the full spectrum of key areas, providing life-changing research, education, and practice. You will serve as a highly valued and respected leader from day one, empowered to make amazing things happen for our patients and our growing success as one of the members of the NewYork-Presbyterian Regional Hospital network.

You will manage complex cases and serve as a strong and knowledgeable liaison between acute care and critical care teams, coordinating the efforts of all disciplines in support of patients and their families. Direct all aspects of inpatient care, ensuring optimal length of stay, anticipate outpatient service needs, and set follow-up appointments and consultations as needed.

Additionally, this role will be the Hospice RN working with Jansen Hospice and Palliative Care, a Certified Hospice Agency, affiliated with  NewYork Presbyterian Hospital This position requires an RN experienced and knowledgeable in hospice care. Also, an understanding of the Medicare hospice benefit, hospice diagnoses, and end-of-life disease processes is essential. You will be a frontline contact for patients, families, physicians, and Hudson Valley Hospital hospice referrals. Responsible for performing on-site nursing evaluations, coordinating hospice services, and collaborating with the Jansen Hospice interdisciplinary team.  This is an opportunity to join a strong, compassionate, growing team of professionals committed to providing quality end-of-life care in patient’s homes, skilled nursing facilities, and hospitals.

This is a full-time days position at NewYork-Presbyterian Hudson Valley Hospital (located at Cortlandt Manor, Westchester County, NY) either 4-10hour shifts or 5–8-hour shifts with rotating weekend requirements.

Preferred Criteria

  • MS highly desirable.

  • Minimum two years experience in Case Management and Hospice experience.

  • Certified Case Manager (CCM) highly desirable.

Required Criteria

  • BSN

  • New York State RN license

  • Ability to function independently and communicate effectively with administrative and professional staff and with people of diverse backgrounds and education.

  • Ability to work with various people and make decisions about issues involving discharge planning, home health situations and available community services.

  • Must possess good understanding of Managed Care/HMO requirements, laws and regulations, Medicare/Medicaid requirements, policy and procedure, and community health, welfare and social agencies policies and procedures.
