Social Work Resource Liaison – Behavioral Health -4S/Geriatrics - Per Diem

Continue the success of our care by collaborating with a network of external programs to effectively implement discharge plans. Secure aftercare appointments coordinate the referral process, and complete applications for community programs and financial benefits.  Escort patients to community interviews for services and arrange patient transportation to ensure a safe transition.  Research available community resources to keep updated and accessible information for diverse patient populations and discharge planning needs.   Build the connections that make the difference for our patients.

This is a per-diem day shift position Monday - Friday ( 8:00 am - 5:00 pm) at our New-York Presbyterian Westchester Behavioral Health Center

Preferred

  • Prior experience working with behavioral health patient population

  • Bachelor’s degree in Social Work, Psychology, or related field

Required

  • Good interpersonal and organizational skills with the ability to prioritize

  • Experience with Microsoft Office suite

  • Basic knowledge of human development

  • Knowledge of basic Social Work skills

  • Knowledge of department policies and procedures

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.
