Position Summary

Reporting directly to the Nurse Manager, the Clinical Coordinator III (Registered Nurse III) assists physicians with patient care, health education/promotion, electronic medical record (EMR) chart management, coordination of patient care, and clinical protocols as needed.

This experienced Clinical Coordinator III (Registered Nurse III) duties will include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, and patient education that can be in person, via our patient portal, or by phone. The Clinical Coordinator III (Registered Nurse III) will work closely with multiple providers and other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values. This nursing position will serve as a key member of the patient care team.

Responsibilities

Administrative/Operations

  • Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician.
  • Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed. Administer immunizations and medications.
  • Executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department.
  • Assumes responsibility and accountability for effectively managing nursing care of individual patients
  • Supports on-going clinical functions of the service to provide compassionate patient-centered care.
  • Handles telephone, My Chart and in-person triage (for symptom management) as well as patient education.
  • Assists with the management of urgent and emergent patient situations.
  • Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed.
  • Reviews and acts on test results (lab, x-ray).
  • Reviews and manages communications from internal and external “customers” inclusive of in basket messages, updating immunizations, updating health maintenance items.
  • Ensures infection control compliance within the practice.
  • Addresses appropriate measures to provide patient and family satisfaction.
  • May float to other clinics on as needed for operations need.
  • May establish standard clinical supply storage in collaboration with other clinical staff and audit that standards are being maintained. Establish standard protocols for medication/immunization inventory management and storage
  • Other duties as assigned and appropriate to scope, education, and/or training.

People

  • Participates in recruitment of clinical staff
  • Precepts clinical staff (medical assistants, licensed practical nurses, and RNs and perform competency testing for new and established clinical staff.
  • Serves as a mentor and resource to others

Specialty Specific

  • Provides cross-coverage nursing support across all clinical areas within the multi-specialty Children’s Center of Excellence, adapting to the unique workflows, patient populations, and operational needs of each subspecialty.
  • Supervises and monitor regulatory items, including emergency medication inventory, vaccine storage, and vaccine tracking in alignment with approved internal protocols.

Compliance/Other

  • Serves on relevant committees, task forces, and workgroup within the department
  • Maintains patient privacy and confidentiality according to HIPAA requirements at all times.
  • Keep current on all organizational and practice policies (e.g., infection control, HIPAA), goals, initiatives, and required trainings
  • Ensure that University and Department policies and procedures are followed.
  • Successfully complete all required university, hospital, and department trainings.

Minimum Qualifications

  • Bachelor of Science degree in Nursing or equivalent education, training, and experience.
  • A minimum of 4 years of related experience. Clinical rotation experience associated with BSN degree programs will be applied toward related experience requirements.
  • An equivalent combination of education and experience may be considered.
  • Current and unrestricted New York State RN license.
  • Basic Life Support certified.
  • Ability to work indoors in an environmentally-controlled environment with a degree of dust, noise, odors, etc., without undue discomfort.
  • Ability to perform sedentary work that primarily involves sitting or standing for long periods of time.
  • Travel between clinical sites may be required depending on need.
  • Duties performed routinely require exposure to blood, bodily fluid, and tissue. Incumbent in expected to perform and/or/assist in emergency care and/or first aid as needed.
  • Ability to work collaboratively with a culturally diverse staff and patient/family population demonstrating tact, respect, and sensitivity.
  • Must have excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management and other stakeholders. Must have highly developed interpersonal skills and ability to deescalate stressful situations.
  • Must be an engaged and positive team member demonstrating collaboration, inclusion, reliability, punctuality, and adaptability to new and changing situations including helping peers adapt to new situations.
  • Strong proficiency in computer skills including, but not limited to, Electronic Health Systems, Microsoft Excel, Microsoft Word, Email systems, etc.
  • Must be a motivated individual with a positive mindset and exceptional work ethic.
  • Participation in medical surveillance is required.
  • Must successfully complete systems training requirements.

Preferred Qualifications

  • Physician practice experience preferred.
  • Previous experience with pediatric populations.
  • Advanced Life Support certified.

Salary

$69,300 - $120,000

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
