This role offers a unique opportunity to practice across the care continuum—managing both acute inpatient needs and longitudinal outpatient care—while working alongside a dedicated team of heart failure cardiologists.

Nurse Practitioner - Advanced Heart Failure Services - Queens Medical Group
As part of the Advanced Heart Failure team, the Nurse Practitioner will play a central role in both inpatient and outpatient care delivery, helping improve transitions of care and long-term outcomes for this high-risk patient population. Duties will include but not limited to the following:
Key Responsibilities
- Provide comprehensive care to advanced heart failure patients in collaboration with heart failure cardiologists and the multidisciplinary team
- Participate in both outpatient clinic care and inpatient service responsibilities
- Conduct inpatient consultations for the Advanced Heart Failure service
- Facilitate outpatient management, including:
- Telephone triage of patient symptoms
- Review and follow-up of diagnostic results and lab findings
- Medication titration and care coordination
- Support transitions of care between hospital discharge and outpatient follow-up
- Participate in the evaluation and workup of patients for advanced heart failure therapies (including mechanical circulatory support and transplant evaluation as applicable)
- Provide care for patients with advanced heart failure devices, including management of patients with remote pulmonary artery pressure monitoring systems
- Deliver care in accordance with Practice Protocols, Clinical Practice Agreements, and institutional policies
This is a 4 days per week, day shift position based in Flushing, NY, with responsibilities across both inpatient and outpatient settings.
Preferred Criteria:
2+ years of Nurse Practitioner experience preferred
Prior acute care RN experience in cardiology strongly preferred
Experience in heart failure or cardiology (inpatient and/or outpatient)
Required Criteria:
Bachelor of Science in Nursing; Master's Degree as a Nurse Practitioner.
Current New York State License as Nurse Practitioner.
Current New York State RN license.
Certification as a Nurse Practitioner from a New York State approved accrediting body – American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP).
ACLS and BCLS Certification from the American Heart Association
Demonstrated critical thinking and analytical skills, with the ability to assess complex clinical situations and develop appropriate care plans
Excellent verbal and written communication skills, with the ability to effectively interact with patients, families, and multidisciplinary team members
Strong team-oriented mindset, with the ability to collaborate closely with heart failure cardiologists and healthcare professionals to achieve shared patient care goals
Ability to manage multiple priorities in a fast-paced clinical environment while maintaining attention to detail
