Manager Clinical Pharmacy- Lung Transplant
NewYork-Presbyterian/Columbia University Irving Medical Center, a premier academic medical center affiliated with Columbia University Vagelos College of Physicians & Surgeons, delivers exceptional, patient-centered care. Within this leading transplant program, the Clinical Pharmacy Manager partners closely with thelungtransplant team to provide advanced clinical pharmacy services, including participation in multidisciplinary rounds, transplant selection meetings, and patient education. Essential Job Duties
Maintains a liaison relationship with the Service covered through participation in patient care rounds and involvementinjointprojectswiththeDepartment/Division/Servicecovered.Thisincludesbutisnotlimitedto:
Identificationofproblems/issuesinspecificpatientsandpromotionofrationaldruguseonacase-by-casebasis (includes therapy selection, monitoring and assessment of outcomes of therapy).
Provisionofdruginformation toproviderscoveringtheserviceandthepatientscovered bytheService(s)identified.
AdverseDrugReactionReportingforpatientsonservice.
Develops,implements,managesandreportsresultspertinenttocontinuousqualityimprovementandexpense reduction programs related to the patients serviced.Examples of such efforts should be:
Medication use valuations.
Treatmentprotocols/guidelinesClinical pathways.
Otherproducts/serviceswhichfulfillthesegoals.
Actsassecretaryormemberofmultidisciplinarycommittee(s)ofmedicalstaff,charged bytheFormularyand Therapeutics Committee or other Medical Board Subcommittee, which oversees, develops, implements and manages policies for the care delivered to the patients serviced.
Providestrainingandeducationalprogramsforundergraduateandgraduatepharmacystudents,universities,and health care providers atNewYork-Presbyterian Hospital.
Assistsintheprocessofbudgetforecastingforpharmaceuticalsbaseduponaknowledgebaseofongoingresearch for new drugs and indications and trending of current consumption.
DevelopsanddeliverseducationalprogramsforpatientscaredforatNewYork-PresbyterianHospital.
Performsresearchinareasofinterest,especiallythose whichparalleldepartmentalgoalsandobjectives,and submits manuscripts for publication and/or presentation at local, state and/or national meetings.
Participates in supervisory functions for pharmacy staff including:
Interviewscandidatesforpharmacymanagementandstaffpositions.
Recommends corrective action for pharmacy staff for supervisor implementation. Contributestoandadministers,whenapplicable,performanceevaluationsforstaff.
Providesguidancerelatedtopharmacotherapydecisions.
OtherdutiesasoutlinedbythePharmacyDirector,DrugUsePolicyofNewYorkPresbyterianHospital, Apothecary-in-Chief of New York Presbyterian Hospital, including departmental Administrator-on-call.
Performsalldutiesinherenttomanagerialrole.Participatesinhiring,training,evaluationandterminationof assigned staff according to hospital policies and procedures.
Performsotherspecialprojectsanddutiesasassigned.
Preferred:
Completion of PGY2 Residency in Solid Organ Transplant
Required:
Completion of a Bachelor of Science degree in Pharmacy degree or PharmD
New York State Pharmacy License or eligibility to obtain license within 3 months
