NewYork-Presbyterian has a long tradition of bringing hope, health, and healing to our communities. NYP is continuously implementing new programs to meet the unique needs of our communities and is strengthening those that exist. Our programs build upon, and tie together, the Hospital’s inpatient care coordination, Ambulatory Care Network, and community outreach programs. Now, we're seeking a Specialist for our Central Post-Acute Resource (PARC) Specialist to help drive this work forward.
The centralized Post-Acute Resource Center (Central PARC) Specialist will support New York Presbyterian (NYP)’s post-acute care strategy. Under the supervision of the Central PARC program manager, collaborate with the interdisciplinary team to enhance transitions throughout the care continuum, and build and manage relationships with post-acute care providers.
Be a part of a team that consistently identifies opportunities to improve patients’ post-discharge experiences and outcomes, and develop strategies to achieve these ends. Build, foster and manage collaborative partnerships with post-acute providers, and facilitate communication of all aspects between key stakeholders. Conduct periodic on-site visits to post-acute providers’ facilities to identify opportunities for mutual improvement, and ultimately improve care transitions.
Preferred Criteria
Master’s Degree
Clinical background (RN, LMSW, LCSW, PT/OT, or other)
Basic knowledge of performance improvement concepts and EMR documentation
Experience working with post-acute care providers (including home care agencies and skilled nursing facilities) and other post-acute resources
Required Criteria
Minimum of 5 years in healthcare environment
Bachelor’s Degree

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
NYP will not reimburse for travel expenses.

Salary Range:
