Social Worker OB - Alexandra Cohen Hospital for Women - 4 days a week

New York -Presbyterian Alexandra Cohen Hospital for Women and Newborns is exclusively dedicated to the unique healthcare needs of pregnant patients and their newborn babies.
We are seeking a Social Worker to join our dedicated team, someone who is passionate about providing high-quality, compassionate care across diverse patient populations with various psychosocial needs. The Social Worker in this role specializes in working with families and provides comprehensive psychosocial assessment, including assessing for potential abuse, neglect, intimate partner violence, mental health and substance use treatment needs. Additionally, the Social Worker provides extensive counseling and linkage to appropriate supports during instances of pregnancy loss or infant loss. In this role you will also utilize your inpatient hospital experience to implement discharge plans for mothers and newborns transitioning onto the next level of care, ensuring continuity of care with optimal patient outcomes. Other responsibilities include:
Promoting a collaborative relationship with the interdisciplinary team to prevent readmissions
Participating in Service Line initiatives to support throughout and patient flow
Serving as a liaison between the hospital and agencies in the community
Maintaining accurate and timely documentation of discharge plans and Social Work plans of care
This is a part-time, four (4) day a week position which includes weekends and holidays. Wednesday - Saturday or Sunday - Wednesday
Preferred Criteria
Prior experience working in Women’s Health
Prior experience with Discharge Planning
Prior experience with Bereavement Work
Prior experience with Child Protective Services
Required Criteria
Master’s Degree in Social Work (MSW)
New York State License as a Licensed Masters Social Worker (LMSW) or New York State License as a Licensed Clinical Social Worker (LCSW)
