Social Worker - Heart Failure/LVAD - Weill Cornell - Days
The Inpatient Social Worker on the Heart Failure/LVAD (Left Ventricular Assist Device) Heart Transplant Service Line plays a pivotal role in ensuring patients receive comprehensive care and support. The HF/LVAD/Transplant SW works closely with an interdisciplinary team, including doctors, nurses, rehabilitation therapists, and dietitians ensuring integrated and holistic care for patients. The role involves conducting comprehensive psychosocial assessments to identify patients' needs, strengths, and areas requiring intervention, developing care plans that address physical, emotional, social, and financial aspects of their lives.
In addition, the social worker also connects patients and families with vital community resources and support services, such as transportation, housing, financial assistance, and meal programs. The social worker provides counseling and emotional support to help patients and their families cope with the psychological impact of chronic illness, hospitalization, and treatment, offering education on stress management, lifestyle changes, and adherence to medical regimens. The social worker also facilitates support groups and educational workshops to help foster a sense of community and shared experience among patients and families.
By providing a comprehensive holistic approach, the Social Worker plays a crucial role in enhancing the quality of life and health outcomes for patients with heart failure, LVADs, and those undergoing heart transplantation.
This is a Monday-Friday day shift position.
Preferred Criteria
At least one year of inpatient Medical/Surgical experience
Discharge Planning experience
Required Criteria
Master’s Degree in Social Work (MSW)
New York State License as a Licensed Masters Social Worker (LMSW)
