Social Worker Bi-lingual Spanish Family Planning Practice and School-Based Health Center ACN Days

Experience a place where your discipline is immensely respected. In this Social Worker role, you will be supporting two community-based clinical practices: the Family Planning Practice and our School-Based Health Center at George Washington Educational Campus. You will meet clients in person addressing behavioral health care within a primary care practice.  You will provide clinical services such as mental health screening, short term mental health counseling, crisis intervention, case management, and patient referrals to community and outpatients services. You will receive same day referrals and have to manage multiple priorities to ensure the best patient experience.

The schedule is Monday – Friday, 8:00 am – 4:00 pm, days will be split between the two practices both located on Audubon Avenue.

Preferred Criteria:

• LCSW

• Bi-lingual Spanish

• Mental health counseling experience

Required Criteria

• Master's Degree in Social Work (MSW)

• NYS LMSW
