Our programs build upon, and tie together, the Hospital’s inpatient care coordination, Ambulatory Care Network, and community outreach programs. Now, we're seeking a Program Lead to help drive this work forward.  

Program Lead  – Grant Administration/Coordination - Columbia - Grant Funded

The Comprehensive Health Program provides an integrated model of care that includes primary medical care, specialty care and essential support services for people living with HIV or at risk for HIV, who are uninsured or underinsured. The CHP works with cities, states, and local community-based organizations to provide HIV care, treatment, and prevention services ( Hep C, PrEP and PEP) to more than 5000 individuals each year. Grant funding is a key resource for the sustainability and diversity of our programming.  Step into this pivotal role, where you will support robust grant programming Care Coordination Services for People with HIV and Outpatient Ambulatory Services for People Aging with HIV.  As part of your work, you will also collaborate with staff throughout the NYP system, and other community-based organizations in Manhattan, Brooklyn, and the Bronx. 

Preferred Criteria

  • MSW/LMSW

Required Criteria

  • Bachelor's degree
  • Minimum of 3 years of experience
  • Experience with HIV Care Coordination and Services for People Living with HIV
  • Experience and/or an interest in community wellness (i.e. peer support, wellness coaching, goal setting, conflict resolution) 
  • Technology savvy to support patients with their use of MyChart for telemedicine visits and virtual engagement in support groups
  • Desire to exceed expectations in supporting program to achieve its quality-of-care goals

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.

$76,000-$99,500/Annual
