Hospital at Home is a unique and innovative program at NYP that provides inpatient hospital level care at the patient’s home. The program combines daily in-home visits, virtual visits, and 24/7 remote patient monitoring to give eligible patients an opportunity to stay in the comfort of their own home while receiving their hospital care. This program will be designed and delivered by NYP providers and clinical staff, allowing NYP's best-in-class care across all measures of quality, service, cost, and innovation to be continued in the home setting. Hospital at Home will provide strategic value to NYP by alleviating capacity constraints, improving patient experience, reducing readmission rates, and driving cost savings to the organization. Since 2020, CMS has authorized more than 320 hospitals across 133 health systems in 37 states to offer acute hospital care at home, and those numbers are climbing. The number of programs is expected to double by 2026. We are looking for motivated team members to help us become the largest, safest, and most innovative Hospital at Home program in the country.
We are currently seeking an Associate Chief Advanced Clinical Provider to lead our Hospital at Home (HAH) Advanced Clinical Providers (ACPs). Under the direction of the Hospital at Home (HAH) Program Director of Operations, Medical Directors, Department of PA Services, and the Supervising Physician, this leadership role combines clinical excellence with operational oversight in the delivery of high-quality, patient-centered care to patients receiving acute-level services in the home setting.
Shift: 24/7 service; three 12.5-hour shift rotation, including nights, weekends, and holidays
Key Responsibilities:
Provide direct supervision and leadership for Advanced Clinical Providers, including oversight of work assignments, scheduling, and clerical functions.
Assist in performance evaluations, address personnel concerns, and recommend disciplinary actions as needed.
Collaborate with clinical and operational leadership to develop and enhance onboarding, training, and educational programs for ACPs, ensuring competency in diagnostic, therapeutic, and procedural techniques.
Foster effective teamwork and communication, promote cross-training and multitasking, and encourage high levels of staff engagement.
Contributes to the development and implementation of policies, procedures, and clinical workflows for the Hospital at Home program.
Deliver high-quality clinical care to HAH patients across emergency departments, inpatient units, and the Virtual Unit.
Manage the full continuum of care for HAH patients, including enrollment, admission, discharge, consultative services, and telehealth visits.
Required Qualifications:
Bachelor’s or master’s degree in physician assistant studies
Current New York State PA licensure
NCCPA certification
DEA registration and current BLS/ACLS certifications
Minimum of 5 years of experience as a Physician Assistant, including at least 2 years in a supervisory role
Preferred Qualifications:
Clinical experience in Inpatient Hospitalist Medicine and/or Emergency Medicine

Salary Range:
