Manager Clinical Pharmacy – Ambulatory Care (Grant-Funded)

Clinical pharmacists within the Ambulatory Care Pharmacy team are integrated into the patient-centered home model to optimize chronic disease management. In this grant-funded position, as Clinical Pharmacy Manager – Ambulatory Care, you'll work closely with the Senior Director of Clinical Pharmacy Services, the Ambulatory Care Pharmacy Director, the Dalio Center for Health Justice, and the digital health team to expand pharmacist-led hypertension management across medical group practices. You'll provide collaborative pharmaceutical care to patients in the outpatient setting while working in conjunction with physicians, nurses, and other healthcare professionals. The Clinical Pharmacy Manager provides direct patient care to patients and conducts teaching and research activities to promote optimal, cost-effective patient care.

The primary responsibilities of this role include maintaining a liaison relationship with the outpatient service covered through participation in collaborative management for hypertension and involvement in joint projects with the Department/Division/Service covered. This includes but is not limited to:

·Completion of virtual and/or in-person pharmacy patient appointments to optimize medication regimen in collaboration with the primary care providers

·Expansion of digital tools and self-tracking measures to assist with hypertension management

·Identification of problems/issues in specific patients and promotion of rational drug use on a case-by-case basis (includes therapy selection, monitoring and assessment of outcomes of therapy)

·Provision of drug information to providers covering the service and the patients covered by the Service(s) identified

·Patient medication education counseling including but not limited to antihypertensives regimens

This role also includes responsibilities pertaining to quality improvement and cost reduction via medication use evaluations, committee work and oversight, and the development/maintenance of treatment protocols, clinical pathways, and guidelines.

Preferred Criteria

·Completion of a PGY2 Residency in Ambulatory Care

·Prior experience in CDTM or an advanced scope of practice

Required Criteria

·Graduate of an ACPE Accredited School of Pharmacy

·Related Masters or Doctoral Level Academic degree and/or Doctor of Pharmacy degree

·New York State Pharmacy License, or eligibility to obtain license within 3 months of hire

·Completion of a PGY1 Pharmacy Residency

$157,000-$193,000/Annual
