Manager Clinical Pharmacy

Hospital at Home (HaH) is an innovative program designed to deliver inpatient level healthcare to patients in the comfort of their homes. By extending hospital-level care to individuals outside the traditional hospital setting, we are redefining how healthcare is provided. This program ensures patients receive the necessary medical treatments while maintaining the comfort and familiarity of their own homes, improving patient satisfaction, and reducing hospital readmissions.

In this role you will maintain a liaison relationship with the HaH department through participation in patient care rounds and involvement in joint projects.  This includes but is not limited to:

  • Identification of problems/issues in specific patients and promotion of rational drug use on a case-by-case basis (includes therapy selection, monitoring and assessment of outcomes of therapy).

  • Provision of drug information to providers covering the service and the patients covered by the HaH program

  • Adverse drug reaction reporting for patients on service.

  • Order verification and coordination of medication dispensing.

  • Conduct and/or oversee medication reconciliation when HaH patients arrive to their homes.

Participate in continuous quality improvement and expense reduction efforts related to the patients serviced.  Outcomes of such efforts should be the development of:

  • Drug use evaluations

  • Treatment protocols/guidelines

  • Care maps/critical pathways

  • Other products/services which fulfill these goals

Preferred Criteria:

  • A general or specialty residency or fellowship are preferred

  • One to three years of previous experience as a clinical manager preferred

Required:

  • Position requires the knowledge normally acquired through completion of an entry level or post-baccalaureate Doctor of Pharmacy program

  • New York State licensure or eligibility for licensure required as a Pharmacist by the State of NY

  • Knowledge of Pharmacotherapy

  • Knowledge of Interpersonal Communications

  • Knowledge of Computer Applications

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.

Salary Range:

$152,000-$199,200/Annual
