Our Division of Community and Population Health programs build upon, and tie together, the Hospital’s inpatient care coordination, Ambulatory Care Network, community outreach programs, and the effort of community-based providers.
Supervisor – Robert Center for Community Health Navigation
Started in 2005, the Center for Community Health Navigation (CCHN) at NewYork-Presbyterian Hospital is dedicated to support the health and well-being of patients through the delivery of culturally-sensitive, peer-based support in the emergency department, inpatient, outpatient, and community settings with the ultimate goals to promote healthcare self-management and to decrease avoidable system utilization.
The Supervisor will recommend personnel actions, including hiring and firing, and prepares written performance evaluations. Establishes baseline standards for meeting performance goals, and monitors and maintains work performance quality standards. May assign and/or schedule work, conducts on-the-job training and answer job-related questions. Assists in the development and implementation of approved systems to provide cost-effective management of departmental activities (i.e., information retrieval, scheduling, and productivity). May retrieve data using department computer system, check output for accuracy and completeness and maintains on-line system support. Makes appropriate recommendations to department Manager to improve systems and department operations. Ensures timely and accurate flow of information to staff and senior management. Also maintains effective communication with other hospital employees in related departments, when necessary to ensure efficient departmental operations. May coordinate department activities with patient care and other hospital services.
Preferred Criteria
- Previous supervisory experience
- Bachelor’s Degree
- Proficient in Microsoft Office: Word, Excel, Power Point, Outlook
Required Criteria
- Associate’s Degree or equivalent work experience

Salary Range:
