Social Worker -  Pediatric Social Worker - BMH - Days

The Pediatric Social Worker is to provide discharge planning for patients admitted to inpatient pediatrics and the pediatric intensive care unit (PICU). Duties include:

• Conduct psychosocial assessment with child and family

• Secure community services that will be needed post discharge such as homecare, medical equipment, outpatient mental health services, or inpatient rehabilitation in a skilled nursing facility. 

• Help parents cope by providing education, counseling, and other helpful resources.

• Identify child abuse and neglect and make reports to the State Central Registry

• Arrange transportation when needed. 

• Work with pediatric psychiatry to develop safe discharge plan

• Obtain psychiatric bed for patients in need of inpatient psychiatric treatment.  This includes getting insurance auth, coordinating with the family, finalizing transfer with NYP Access Center.

• Participate in interdisciplinary rounds

Shift:  M-F, 9-5, with one weekend per month; 1199 position

Preferred Criteria

• Experience as an inpatient social worker

• Experience working with pediatric patients

• Experience using Epic and Allscripts

Required Criteria

• Masters Degree in Social Work (MSW)

• New York State License as a Licensed Masters Social Worker (LMSW)
