As a top ranked hospital in New York by U.S. News & World Report, New York-Presbyterian (NYP) has a long tradition of bringing hope, health, and healing to our communities. NYP’s commit­ment to addressing the specific needs of its local communities shines brightly through the work of its Division of Community and Population Health. The Community Service Plan (CSP) provides a roadmap for continuing the Division’s work to provide comprehensive outreach, engagement and treatment for community members living with HIV and/or at-risk of HIV. To address social determinants of health and mitigate risk for poor outcomes among vulnerable populations, the new CSP HIV team has embraced comprehensive care coordination strategies and practices multidisciplinary, team-based care including integrated behavioral health services, intense care coordination and primary care.

Program Coordinator – Comprehensive Health/Care Coordination – Supporting Brooklyn Methodist Hospital- Grant Funded

Under the guidance of the Program Manager, the Program Coordinator will be responsible for working with frontline care providers and care coordination staff to promote HIV testing. Ensure that frontline staff is given daily reminders of the presence of Care Coordinators and the importance of providing HIV testing and linkage to the STI clinic and Nurse Practitioner for prevention and treatment services. Using the dashboard to assess emergency department patients for potential symptoms of STIs and HIV and reach out to providers to encourage appropriate testing. Work directly with staff and clients in the community or at Brooklyn Methodist Hospital or mobile medical unit (MMU), Participate in daily and weekly multidisciplinary case conference with program staff from NYP and associated Community Based Organizations (CBOs). Serve as main point of contact for CBOs for low-threshold linkage of their clients to HIV care or prevention services at NYP or at the program of a client’s choice, work to link clients to primary care and Health Home services, if indicated. Meet directly with clients to complete intake into supportive programs, if indicated. Provide outreach and linkage services to selected patients testing positive for HIV, HCV or in need of prevention services. Provide behavioral and adherence counseling for clients receiving HIV treatment or prevention services. Additional administrative and coordination activities as deemed necessary by program leadership

Preferred Criteria

  • Master’s degree
  • Bilingual in Spanish

Required Criteria

  • Bachelor's Degree or minimum of three (3) years of equivalent work experience
  • Excellent engagement skills
  • Strong interpersonal skills, highly organized, ability to work with diverse community groups and multi-task
  • Passion for working community members living with HIV and/or at-risk of HIV

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

Salary Range:

$58,500-$79,000
