In this role, the Patient Navigator functions as the primary care coordinator for patients and collaborates with key clinical staff to ensure comprehensive care and treatment. Coordinates appointments, provides logistical support, acts as a liaison between the patient and medical team, directs patient inquires to appropriate staff, and facilitates collection of medical records, insurance and financial information. Position includes direct contact and communication with patient, in person, on telephone or via e-mail.
This is a full-time day shift position, located at NYP/Brooklyn Methodist Hospital.
Preferred Criteria:
- Bachelor’s Degree from an accredited college or university.
- Experience in health care setting working directly with patients.
- Bi-lingual in Cantonese, Mandarin, Spanish or Russian.
Required Criteria:
- High School Diploma or GED
- Excellent oral and written communication skills.
- Diverse and culturally sensitive customer service skills.
- Ability to manage high stress, chronically ill population with complicated psycho-social issues.
- Basic computer skills (Microsoft Word, Excel).
