Whether an individual faces a possible cancer diagnosis, or has been diagnosed with cancer, it is very stressful. Beyond questions and concerns about the disease, questions may arise such as “what do I do next?” “How am I going to get to my appointment?” and “how am I going to pay for this?”  The good news is NewYork-Presbyterian Brooklyn Methodist Patient Navigators have answers to these questions and provide solutions and guidance to our patients during their cancer care experience.

Patient Navigator - Breast Center – Grant Funded - Days

The Patient Navigator role functions as the primary care coordinator for Breast Center patients and collaborates with key clinical staff to ensure comprehensive care, follow up, and treatment.  Responsibilities include determining eligibility and enrolling patients as necessary, coordinating appointments, providing logistical support, facilitating the collection of medical records, insurance and financial information while acting as a liaison between the patient and the medical team.  In addition, the Patient Navigator helps to identify barriers that may be preventing patients from receiving timely and high-quality healthcare treatment and working to find solutions to those barriers. The Patient Navigator will also participate in/coordinate outreach events and education sessions to ensure the local population of Brooklyn has the resources for cancer screenings, not limited to breast cancer.

This role is an essential part of the Breast Center and Oncology Service Line and will report directly to the Manager – Oncology Service Line.

This is a full-time Monday – Friday 9:00AM – 5:00PM day shift position located at NewYork-Presbyterian/Brooklyn Methodist Hospital. Flexible hours for outreach and community events.

Preferred Criteria

  • Bachelor’s Degree preferred
  • Bilingual language skills (Mandarin and Cantonese)
  • Experience in a healthcare setting working directly with patients
  • Experience with Health Education and Health Outreach in the community

Required Criteria

  • High School Diploma or GED
  • Demonstrates an understanding of population-based disease management including community resources
  • Demonstrates an ability to function independently in a goal directed manner while communicating effectively with patient, family, significant others and providers at all levels
  • Demonstrates sensitivity to patient/family belief systems, ethnicity and culture as well as socioeconomic background

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.

Salary Range:

$21.35 - $34.15/Hourly
