The Care Manager intervenes and provides support to patients who have complex behavioral health needs. The Care Manager will be responsible for identifying at-risk populations and receive referrals from interdisciplinary team members, including providers, Registered Nurses, and other members of the care team. In addition, will conduct managed care and utilization management reviews for all levels of care. Ensures that patients are transitioned appropriately throughout the continuum of care according to established criteria of care. Actively collaborates and consults with treatment teams and external agencies. Receives clinical and administrative supervision from the Manager of Patient Case Management Department. Supervision consists of an annual performance appraisal interview, and weekly group and/or individual meetings as necessary. Frequent contacts with employees, patients and families, faculty, Managed Care companies and various external agencies.

This is a full-time day shift position 9:00am-5:00pm Monday-Friday, located at NewYork-Presbyterian Brooklyn Methodist Hospital.

Preferred Criteria:

  • Bachelor’s degree in nursing or Master of Social Work (MSW) Degree.
  • Licensed Master Social Worker by New York State Education Department or Current New York State License as a Registered Professional Nurse.

Required Criteria:

  • Bachelor's degree
  • A minimum of 2 years of experience in mental health field after completion of degree.
  • Experience with a broad range of psychopathology and substantial understanding of the phenomenology, dynamics and treatment of the psychiatric population within current behavioral health care context.
  • Capacity to conduct case certifications and reviews to present rationale for level of care, LOS, interventions, and discharge plans. Ability to deliver these functions for all payer sources and across all levels of care.
  • Ability to negotiate on behalf of patients within the context of managed care constraints.
  • Knowledge of reimbursement protocols and billing issues pertaining to all payer sources.
  • Capacity to collaborate with interdisciplinary staff, Patient Finance department, Medicare and other external agencies.
  • Ability to demonstrate excellent communication and strong relationship building skills.

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.

Salary Range:

$58,500-$90,000/Annual
