Lawrence Community Health Services (LCHS) brings together an interdisciplinary team of nurses, social workers, therapists, and a Medical Director who works collaboratively to support patients and families throughout our community.
The Documentation Improvement Specialist plays a key role in ensuring accurate, high‑quality, and compliant clinical documentation across both Home Care and Hospice. Under the supervision of the Director of Patient Services, this role pre-codes referrals, reviews admission documentation, approves Plans of Care, collaborates with clinicians to improve accuracy, and supports weekend Intake operations. The specialist provides guidance, education, and feedback on OASIS and HOPE accuracy, regulatory requirements, and documentation best practices. This role supports key quality and compliance initiatives while serving as a clinical resource to interdisciplinary staff. This is a hybrid role, based at Lawrence Community Health Services, with a combination of on‑site and remote work as appropriate.
This is a full-time day shift opportunity that includes Weekends: Saturday: 8:00 AM – 2:00 PM and Sunday: 8:00 AM – 4:00 PM, On Weekdays (Hybrid): Monday: 10:00 AM – 5:30 PM, Tuesday: 10:00 AM – 6:00 PM, and Wednesday: 10:00 AM – 6:00 PM. Days Off: Thursday & Friday.
Key Responsibilities
- Perform concurrent and retrospective documentation review to ensure accuracy, completeness, and regulatory compliance.
- Review and approve Plans of Care for Home Care and Hospice.
- Collaborate with nurses, physicians, and other licensed independent practitioners to clarify and improve documentation.
- Support weekend Intake by reviewing referrals, coordinating weekend admissions, and communicating with clinical teams and hospital partners.
- Provide education and feedback to staff on documentation quality, OASIS scoring, ICD‑10 accuracy, and regulatory requirements.
- Assist with monitoring documentation trends and supporting quality improvement initiatives.
- Ensure documentation supports coding accuracy, service line metrics, and organizational goals.
Preferred Criteria
- If assigned to Rehabilitation: Current New York State Occupational Therapist or Physical Therapist License
- Home Care Oasis Certification
- ICD Coding certification
Required Criteria
- Bachelor’s Degree in Nursing
- If assigned to Rehabilitation: Bachelor’s Degree in Occupational Therapy or Physical Therapy (if not a Nurse with experience in the provision of rehabilitation services)
- Current Registered Nurse, Nurse Practitioner, or Physician Assistant license highly preferred
- 5 years of recent hospital acute care experience and/or verifiable Documentation Improvement experience
- Strong organizational, verbal, and written communication skills, orientation to accuracy and detail, and ability to relate to a variety of professional staff are essential

Salary Range:
