Clinical Coordinator Field RN – Home Care - Days

This position requires an RN experienced and knowledgeable in Home Care. Also, an understanding of the Medicare home care benefit, OASIS documentation, and Home Health Value Based Purchasing. You’ll be a frontline contact for patients, families, field clinicians, and physicians. Responsible for coordinating patient services and communicating with colleagues in the field. This is a full-time day shift opportunity working 5 days a week, working 8:30am-4:30pm. Will be required to work some weekends and holidays. The location for this role will be covering Westchester County. Responsibilities include but are not limited to coordinating patient services and plan of care, serving as a point of contact for field clinicians, developing educational content for field clinicians, collaboration with the interdisciplinary team. This position is located at Lawrence Home Care of Westchester, a Joint Commission Accredited Certified Home Health Agency.

Preferred Criteria

  • 3-5 years of RN experience
  • Ability to be flexible, apply critical thinking skills, and function independently.
  • Capability to work as a strong, committed member of our homecare interdisciplinary team of professionals.
  • Must have two years of experience in a home care agency.
  • OASIS certification or willingness to obtain

Required Criteria

  • Bachelor's degree in nursing
  • Current New York State RN license 
  • BCLS certification (American Heart Association)
  • Must have a Driver’s License and a reliable car
  • Excellent communication skills and computer proficiency required.

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.
