Hospital at Home allows NewYork-Presbyterian patients to recovery and rest in the comfort of their own home, while still benefiting from the technology and expertise of a NewYork-Presbyterian hospital. This program helps to reduce emergency department crowding and is also associated with a better experience for both patients and providers.
The program combines daily in-home visits from health care providers, virtual visits, and 24/7 remote patient monitoring. Patients and caregivers have access to a clinical team 24 hours a day, 7 days a week. If a patient’s condition changes, the care team can respond quickly and, if needed, arrange a seamless return to the hospital.
