Discharge Planning Coordinator – Case Management
When an individual is preparing for discharge from the hospital, it is very stressful. Questions may arise such as “what happens next?,” “who is going to care for me?,” and “how am I going to pay for this?” The good news is NewYork-Presbyterian Queens Discharge Planning Coordinators have answers to these questions, and provide solutions and guidance to our patients as they prepare to leave our facility and help ensure they receive appropriate aftercare.
The Discharge Planning Coordinator provides a variety of tasks associated with discharge planning and facilitates the transitions of patients to the next level of care. In addition, the Patient Navigator helps to identify barriers that may be preventing patients from receiving timely and high quality healthcare treatment upon discharge and working to find solutions to those barriers.
Day shift with rotating weekends and holidays.
Preferred Criteria
Bachelor’s Degree
Required Criteria
High school diploma or GED
1 year of experience in a healthcare setting
At least one year of experience in a healthcare setting is required.
Strong interpersonal skills, willingness to learn and ability to work in a fast paced and changing environment
Knowledge of Microsoft Office products and general computer literacy Excellent communication skills, both oral and written.
Detailed oriented, good organizational skills and ability to multi-task
Ability to work independently

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
