Social Work services on the Oncology units help patients, families, and caregivers deal with the experience of facing cancer. The Social worker is skilled to assist with the psychological, social, emotional, and spiritual issues patients experience when fighting cancer. Social workers are there to help patients with practical needs, such as finding resources in the institution and the community, and with such complex needs such as adjusting to an illness, dealing with transitions, decision-making, navigating cultural issues, and communicating with family members, friends, and health care providers. The Social Worker aids in the care, development, education and support of all patients and families during a vulnerable time in the lives of patients and families. The patient population on oncology units are a combination of acutely complex oncology patients and medicine off service . The Social Worker on the Oncology Service is responsible for completing assessments, educating and supporting the patient and family in identifying the best discharge plan in order for the patient to progress in the next level of continuum of care. The goal is to provide the highest quality medical care and transition of care for patients. The Social Worker is responsible for facilitating patient access and providing compassionate patient focused care. The Social Worker plays a key role in the process of developing comprehensive discharge plans, beginning on the day of admission and through the continuum of care.
This is a Monday - Friday 9am-5pm position with weekend and holiday rotations.
Preferred Criteria
Inpatient discharge planning experience
Experience working with Oncology patients
Required Criteria
Master’s Degree in Social Work (MSW)
New York State Licensed Clinical Social Worker (LCSW) or New York State Licensed Masters Social Worker (LMSW)
