The Social Work in the ED will work with admitted patients to facilitate discharge. Often these are low acuity patients which may be able to be safely discharged with community support of skilled resources, benefits, education, advocacy or increased family support.
The social worker will mitigate family or resource barriers to discharge working collaboratively with the interdisciplinary team to develop a safe discharge plan, averting transfer to an inpatient unit and a longer length of stay.
The Social Worker will perform assessments to identify patient care needs, establish patient/family expectations, and explore post discharge resources and benefit eligibility patients requiring admission due to medical acuity or being unsafe in the community, the .
Additionally the social worker is cross trained to provide coverage in the adult and pediatric ED the event of unexpected staff absences or during a surge.
This is a Mon-Fri 11:00am-7:00pm (occasionally the shift schedule may vary) (no weekends, no holidays)
At least one year of inpatient Medical/Surgical experience
Discharge Planning experience
Master’s Degree in Social Work (MSW)
New York State License as a Licensed Masters Social Worker (LMSW)
If a new graduate Social Worker, Limited Permit (must supply exam date prior to start of employment)
If a new graduate Social Worker, must have Licensed Masters Social Worker (LMSW) by the State of New York within 1 year of hire
