As part of our growing behavioral health care team, you will experience a place where your discipline is immensely respected. In this Social Work Care Manager role, you'll provide comprehensive care coordination for psychiatrically and medically complex patients throughout the Ambulatory Care Network.  Working alongside a collaborative ambulatory care team, you'll guide patients through the medical system while providing disease specific education and enhancing patient outcomes.  Here, you will combine a comprehensive, holistic approach to address and decrease patient barriers to healthcare engagement and self-care as you coordinate resources and referrals with community based providers.  Drive your career forward while experiencing all of the reasons why you entered such a rewarding field.

This a day shift, Monday - Friday position.

Preferred Criteria

  • Experience working with adult psychiatric patients
  • Fluency in Spanish

Required Criteria

  • Master’s Degree in Social Work (MSW)
  • New York State License as a Licensed Masters Social Worker (LMSW) 
  • If a new graduate Social Worker, may be hired on Limited Permit (must supply exam date prior to start of employment)
  • If a new graduate Social Worker, must have Licensed Masters Social Worker (LMSW) by the State of New York within 1 year of hire
