Social Worker Acute Rehab Unit Weill Cornell Days

Our compassionate, fast-paced rehabilitation center seeks an experienced Social Worker with a background in discharge planning, managing crisis, supportive counseling and able to refer to community resources.

Social workers meet regularly with patients at Weill Cornell Rehab Unit during their hospital stay to address any concerns related to the emotional and physical impact of an illness or disability.

Through a combination of individual, family, and group treatment sessions, the social worker provides support and assists the patient and family in adjusting to the new needs and challenges.

Social Workers also serve as links between the hospital and the community, coordinating at-home care for patients, and encouraging the patient and family to actively participate in formulating a discharge plan that meets your immediate needs. Social workers also discusses planning  long-term recovery.

A social worker identifies any financial concerns patients and families may have, and work with insurance carriers and community resources to ensure the smooth delivery of healthcare services.

Preferred Criteria

  • At least one year of inpatient Medical/Surgical experience
  •      Discharge Planning experience

Required Criteria

  •        Master’s Degree in Social Work (MSW)
  •       New York State License as a Licensed Masters Social Worker (LMSW)
  •       If a new graduate Social Worker, Limited Permit (must supply exam date prior to start of employment)
  •      If a new graduate Social Worker, must have Licensed Masters Social
  • Worker (LMSW) by the State of New York within 1 year of hire
