Patient Navigator - WALLE Program - Grant Funded - Day Shift

Bring your talents to NewYork-Presbyterian, and help drive the success of our Division of Community and Population Health's WALLE Program. The WALLE Program aims to help patients and patients’ caregivers address and prioritize their social needs in the waiting rooms of our clinics and emergency departments. The members of our team would like to achieve the following objectives: 

  • Connect patients to community resources.
  • Inform care team members of patients’ needs as identified by patients upon the completion of screening tools.
  • Support social determinant and clinical screening processes in a variety of clinical settings

In this grant funded role under the direction of the Program Manager, the patient navigator will support screening efforts at various clinical sites while maintaining regulatory compliance provided by the institution.  The patient navigator will ensure that patients complete screenings and communicate with patients, in person, on by telephone, or via email.  The patient navigator will ensure the proper use of technology for all screening operations while maintaining and keeping updated data that will inform the progress of screening efforts., health disparities are prominent among community members of poor socio-economic status. Challenges such as unstable housing, unsafe neighborhoods, low income, or substandard education impact how patients interact with their local health care systems and regularly stand in the way of effective disease management.Social and environmental determinants constitute 50% of factors necessary for good health, while only 20% is attributed to direct clinical care.Now, we’re inviting you to join our team to help alleviate the impact of social determinants of health among the patients we serve in Northern Manhattan and The South Bronx.

Preferred Criteria

  • Fluent in Spanish 
  • Experience in a community healthcare setting
  • Bachelor's degree or equivalent work experience

Required Criteria

  • High School diploma or GED
  • Diverse and culturally sensitive customer service skills
  • Microsoft Word and Excel skills
