In this critical role, you will ensure an exceptional continuity of care for our DHK Infusion Center patients, with a focus on our Head & Neck patient. Take your career in Oncology to the next level, as you assume responsibility for coordinating and tracking patients' progress. Maintain regulatory compliance and collaborate with the Nursing team to implements all Quality Assurance and Performance Improvement initiatives. As a Clinical Coordinator / Nurse Navigator you will utilize your clinical expertise to troubleshoot issues and provide direct patient care. The head and neck oncology nurse navigator (ONN) is a unique and critical position to help patients, their families, and their caregivers as they face their diagnosis and undergo treatment. The expectation is to provide a foundation for helping patients throughout their continuum of care. Coordination of care, open and productive communication, and educational strategies guide the professional behaviors of the Oncology nurse Navigator. Identification and attention to the individual’s barriers to care, education, referrals, and patient-centered care are all important aspects of the interactions of the ONN with their patients.
This is a full time position at New York-Presbyterian Hospital / David H. Koch Center, working Monday - Friday.
- Oncology Certification (OCN)
- ONS chemotherapy / biotherapy certified
- Current NYS RN Licensure
- At least 5 years of experience
- Strong commitment to providing an outstanding patient experience
