Care Manager – RN – Care Transitions – Columbia - Days

The CARE MANAGER - RN is a talented nurse who provides intensive care management for patients who have chronic medical conditions and complex psychosocial needs. The individual will be part of the Transitions of Care team in the Division of Community and Population Health (DSRIP), working to support high-risk patients transition from hospital to community.This individual will assist intake and clinical triage, as well as manage a caseload of patients over time, link patients to NYP and community and resources, and provide self-management support to improve health outcomes.

The Care Manager will engage with patients both telephonically and face to face by making community and hospital visits, accompanying patients to appointments as needed, as well as coordinating extensively with the interdisciplinary care team. He or she will visit patients in the community within Manhattan, the Bronx, and other Boroughs as appropriate.

Care Manager RN iresponsibilities will involve clinical intake and triage, developing individualized care plans, and collaborating with an interdisciplinary team, including physicians, to provide a comprehensive approach to care. Help minimize and prevent hospital readmissions throughout our health system for patients of special needs. Guide patients as they navigate the complex world of screenings, treatments, and recovery. Follow up with patients and their families to ensure their understanding of the plan of care, and educate on how to maintain appointments, DME, and medication. Form strong and enduring bonds with patients, families and multidisciplinary colleagues.

This is a Monday - Friday day shift.

Preferred Criteria

Leadership experience in either inpatient care management, community care management, or outpatient/ambulatory care case management

Experience with long term case planning and developing care plans

Experience working with patients via community visits

Required Criteria

5 years of health care experience

3 yrs of case management in an inpatient, HMO, Insurance or other outpatient setting

Creative thinking and excellent interpersonal skills

Bachelors degree in Nursing (BSN)

Current NYS Registered Nurse (RN) license
