Nurse Practitioner - FNP

Location: NewYork-Presbyterian/Gracie Square Hospital

Summary: 

The Nurse Practitioner at Gracie Square Hospital must provide direct clinical care including assessment, medication management and medication administration. Must possess ability to work autonomously, collaborating with physician, peers and others in the health care community. Successful candidates should have experience working with psychiatric clients as well as with co-occurring substance abuse disorders. Must also have basic computer skills to utilize electronic medical records, emails and word documents. 

 

Essentials Job Duties: 

Must provide direst clinical care, provides treatment planning recommendation as well as Medical management. Participate in clinical training and staff development. Ensure documentation meets regulatory and accrediting agency standards. Provides assistance with administration work. The Nurse Practitioner is responsible for the management of care for a specific group or patients in accordance with mutually agreed upon protocols and within policies and procedures established by the Dept. of Nursing, the American Nurses Association and Gracie Square Hospital. 

 

-Patient Care Management and Documentation: 

  • Obtains and documents subjective history (interval or comprehensive) as appropriate to chief complaint or reason for visit. 
  • Conducts regular physical exam, documenting pertinent positive and negative findings. 
  • Identifies a diagnostic impression and/or formulates a differential based on the subjective and objective data obtained. 
  • Writes order(s) according to New York State and federal law. 
  • Formulates goal-directed plan that includes health maintenance, anticipatory guidance and appropriate medical and nursing interventions according to established protocols/practice agreement. 
  • Asses and documents patient response to treatment interventions. 
  • Provides and documents health teaching that is oriented to the appropriate problem and/or developmentally age-appropriate health maintenance or anticipatory guidance issue. 
  • Consults and/or refers as per scope of practice. Documents same as well as documents follow-up. 

-Quality Assurance: 

  • Conducts review of patient records in collaboration with peers and physician colleagues to determine quality and appropriateness of care provided. 
  • Engages in ongoing quality review activities, competency evaluations, hospital performance improvement initiatives and Medical Board reviews. 

-Patient Safety: 

  • Administers care in accordance with safety, environmental and infectious control principles and regulations identified by the institution and legislative bodies. 
  • Practices in accordance with their scope of practice as delineated by practice protocols and delineation of privileges to include ordering and/or administering medication, Intravenous fluids, blood and blood products. 

 

-Patient Rights: 

  • Demonstrate an awareness and knowledge of patient's rights and responsibilities while receiving care within the institution. 
  • Demonstrates knowledge of the referral mechanism when a patient believes these rights have been violated. 
  • Demonstrates knowledge of the hospital's policy governing the application of the supportive devices to patient. 
  • Protects confidentiality and privacy. 

-Education: 

  • Serves as a resource person and participates in staff development programs. 
  • Provides patient education and counseling on health status and treatment plan. 
  • Serves as clinical preceptor for students from selected programs. 

-Research Studies: 

  • Assists staff in developing appropriate quality improvement study design for potential use in research. 
  • Participates, when appropriate, in quality improvement activities. 
  • Identifies need for investigation and data collection to improve patient care. 
  • Evaluates and implements relevant research findings. 

-Coordination/Collaboration: 

  • Collaborates on clinical problems with nursing service personnel, medical staff or other health care providers. 
  • Collaborates with other members of the interdisciplinary team to determine and achieve health goals for patients and families. 
  • Collaborates with administration and professional staff of the hospital in the establishment and implementation of community services. 

-Professional Practice: 

  • Participate sin departmental staff meetings and committees as requires and appropriate. 
  • Serve as a professional role model for advanced practice staff, the interdisciplinary team and students. 

-Service Excellence: 

  • Treats both internal and external customers with courtesy, respect and caring behaviors. 
  • Promotes a safe therapeutic environment for patients, families and staff. 
  • Acknowledged cultural differences that exist among staff and patients. 
  • Identifies customer service needs on the unit.

  • Performs other job related duties as assigned. 

Education/Licensure/Certification: 

  • Master's Degree in Nursing or completed alternative certification requirements for graduates of nurse practitioner programs prior to April 1,1998 (Required), BSN and/or MSN preferred 
  • National Board Certification in specialty area. 
  • Must possess New York State prescriptive rights 
  • Current New York State Registered Nurse License
  • Current New York State License as Nurse Practitioner in area of practice
  • National Certification as a Nurse Practitioner in the specialty of practice
  • Basic Life Support Certification for all Nurse Practitioners who provide direct patient care
