Care Management Coordinator - Physician Services Division


Step into NewYork-Presbyterian Hospital and help drive the success of the Physician Services Division. You have the opportunity to transform patient care in our NYC boroughs.


As a Care Management Coordinator in the Physician Services Division, you will be an integral member of a collaborative care team that involves the patient, the patient’s Primary Care Physician, and consulting Physicians.  In this role, you will coordinate and support patient care throughout the community, and provide evidenceā€based clinical education to help the patient maintain optimal quality of health.  Help guide patients and families through the plan of care, set up appointments, secure DME, and medication.  Under your guidance, our patients will be empowered to maintain their wellbeing.



Preferred Criteria

  • Leadership experience in either inpatient care management, community care management, or outpatient/ambulatory care case management
  • Master's Degree in healthcare related field
  • 5 years of nursing experience (three of which were in an acute care setting)
  • At least two years of experience in case/care management in an inpatient or outpatient setting, or Health Plan
  • Fluency in Spanish 


Required Criteria

  • Bachelors degree in Nursing (BSN) or Licensed Practical Nursing
  • Current NYS Registered Nurse (RN) licensure or Licensed Practical Nurse (LPN)
  • At least 5 years of health care experience
  • Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical
